How to Become a Patient
Your insurance company may require that you get a referral from your Primary Care Physician before becoming a patient. Please check your policy. If required, you must get the proper authorization from your insurance company before scheduling your appointment.
Scheduling Your First Appointment
If a referral is required, the referring physician may schedule the appointment for you; or, once you receive the referral, you may schedule the appointment yourself by telephoning our office.
We would like to welcome all new patients to our office! Please print and complete the new patient packet. You can either bring it with you to the appointment, drop it off or fax the packet to us at (903) 675-1838 (Athens) or (903) 723-3305 (Palestine).
If you have any questions please call our office (903) 675-2222.
